Setting up pre-paid card with Amazon payments account.

Hi, I'm new to MTurk and I have a Wal-Mart GreenDot prepaid debit card that I'm trying to put on the account. I know where to find my routing and account number but when Amazon asks for a bank statement file and to load it to them I didn't know what to do. I just went to the walmart money card website and saved my most recent transaction statement in pdf format but I don't think it has all of the required information that Amazon wants. Also, I saved a direct deposit form to pdf format but I can't send that to them because I'm still waiting for verification from the other form. So my question is, am I doing the right thing? Can anybody that has dealt with this before help me out?

Just send them an email. I had that happen with one of my cards. I explained that I don't have the document and they accepted the amount of the 2 small transfers.
