A screener HIT is just like any other HIT. Create a HIT with questions that ask about what you need to screen for (maybe you're looking for people in their 30s so your HIT asks everyone for their age) and workers will submit their responses. Take the responses that match what you're looking for (everyone who submitted an age in their 30s) and use those workers for the future HITs you have.
