More than likely you have gotten blocked before, a lot of survey people do it when they have no idea what they are doing with qualifications. I I wouldn't worry unless they blocked you cause you're were doing really, really bad. (Like scammery bad)

Some workers will start at a "reviewer level 2" and some will start at "reviewer level 1" depending on how well you did on the training HIT. I'll usually try to promote the 1s to 2s after a few HITs once it seems like you have the hang of the HIT. However, if your audio or video quality is poor, I might not promote you. Typically the reviewer level 2s get promoted to a 3 after they have completed about 10 HITs. Try your best on your next video and then message me through mturk and I'll either promote you or give you feedback about what you need to be doing better.
