Likert Scale HELP

Hello, I am will be creating a survey with a bunch of Likert Scale questions. The sample code I see on AMT has a generic answer of "answer 1 - 10 if you like birds".

How can I make it so users can only click a series of 5 answers... such as
1. Strongly Agree
.
.
.
5. Strongly Disagree

You would have to code all of that in yourself, making the radio buttons for each question. I think it might be a lot easier to use a 3rd party site for creating surveys, like Qualtrics, SurveyMonkey, Google Forms, etc, then include the link to the survey in your HIT. I believe there's already a HIT template for doing it that way, as well.
