Same for me. I have proof. I just emailed them and politely informed them that if the rejections are not reversed I will contact mturk. I neglected to inform them that I will also post reviews.

Calling requesters, the people/entities who pay you, names could easily be considered offensive/abusive at the very least. That is the very first bullet point in the guidelines. If you would like to discuss it further, you should PM.

Not paying people when you don't provide a code is fraud. It might be a mistake, but it is prima facie fraud.
