A healthcare company named Joany has a survey on their site which takes about 10 minutes and pays $25 via Paypal (I took it on Monday and the payment is already in my account). It is for people who get their health care coverage individually and not through their employer.
You will have to upload a screenshot of your healthcare card but other than that, it is standard survey questions.
